Does Methadone Work?

  1. The thing about this video is that it's somewhat misleading. She says that methadone patients "have to go to a clinic every day to get their dose". That is untrue. In early treatment, yes. However, after 90 days of treatment, if the patient is doing well ( no positive urines, no criminal activity, etc) they begin to earn take home doses. By one year in treatment, they can earn as much as a week's worth of take homes at a time. In many states, by two years they can earn up to one month of take homes. This is the same amount that can legally be given of ANY schedule 2 narcotic at one time, even from a doctor's office.

Additionally, Buprenorphine does not work well for many pts. in MMT. The reason for this is it's ceiling effect. It can be increased up to 32mgs, but after that level there is no further effect gained by increasing it more. Therefore, Bupe tends to work best for those with low level, short term addictions. Patients who require more than about 60mgs of methadone to stabilize are unlikely to do well on Bupe--and the average needed dose of methadone is 80-120mgs.

If a person had been abusing Vicodon pills, say 10-20 per day, for 6 months, Bupe might be the best choice. However, for those who have spent years heavily abusing heroin or Rx opioids in large amounts, it is unlikely to be enough to control their symptoms.

  2. Thank you Zenith15 for any insight you can give on methadone treatment. When my daughter was in the methadone treatment program, she was doing great. She wasn't using drugs and was starting to get her life together until she was forced out of the program by DCYF and the family court in order to regain custody of her newborn daughter. The courts need to realize that a person can not be forced off treatment and it takes longer to detox than thirteen days. They set her up to fail and like any recovering addict she did. Well their happy. They got to auction off her daughter!
