Forty children died in 2011 while under the state’s watch, including five youths who were gunned down, seven who committed suicide and a dozen babies — some sleeping in an adult’s bed — who were deemed victims of sudden and unexpected infant deaths, according to an alarming report one outraged lawmaker called “troubling and frustrating.”
